Treasurer Forms

Reimbursement form- if you made a purchase for a PTA event using your own money you can get reimbursed.  Please note PTA does not reimburse for sales tax.  Complete this form and submit along with your receipt to the treasurer.

Disbursement form- if you are requesting a check from PTA to pay another party for goods/services.  Please submit this form along with the invoice from the seller to treasurer.

Please contact the president or treasurer if the retailer requests a tax exemption certification form.  We can help you complete this form.
